PCN’s are a group of GP Practices that work together to offer integrated healthcare services for the local communities. All practices within a PCN remain independent but share funding to provide staff roles including:

• Social Prescribers
• Health and Wellbeing coaches
• Clinical Pharmacists
• Physiotherapists
• Nurse Practitioners
