Welcome to our website!

Moreton & Meols and Wallasey Wellbeing Primary Care Networks (PCNs) are a group of 8 GP Practices working together with other healthcare organisations to improve the health of our population and address health inequalities.

By collaboratively working together as 2 Networks, we are able to collectivise and utilise our resources and workforce to help tackle health inequalities in the surrounding areas. As 2 PCNs we are able to access additional funding for a wider variety of specialist roles. This helps us to address many of the components that influence health, that GPs could not confront previously due to resource constraints.

The 8 practices that form our 2 Primary Care Networks are:

  • Moreton Group Practice
  • Moreton Medical Centre
  • Hoylake Road Medical Centre
  • Hoylake and Meols Medical Centre
  • Central Park Medical Centre
  • Manor Health Centre
  • Liscard Group Practice
  • The Village Medical Centre

In addition to the eight practices above, we work with other organisations in the wider health community and voluntary sector. This includes:

  • Citizens Advice Bureau – who provide our Social Prescribing Link Workers
  • Health Junction – who provide our Health and Wellbeing Coaches
  • Cheshire and Wirral Partnership NHS Foundation Trust (CWP) – who provide our Mental Health Practitioners
  • Wirral University Hospital NHS Foundation Trust (WUTH) – who provide our Clinical Pharmacists and Pharmacy Technicians
  • Wirral Community Trust – who provide our Community Matrons, Nurse Practitioner for Older People (NPOP) and Early Intervention Assistant (EIA)
  • Pure Physiotherapy – who provide our First Contact Physiotherapists

Working alongside our partner organisations, we also have an established team of ARRS healthcare professionals who utilise their combined expertise to deliver services which help to improve the health of the local population.
